Additional Terms – Security Officer Panel

In addition to the MEDHOST Customer Portal Web Site Terms of Use, these Additional Terms apply regarding your access and use of the Security Officer Panel portion of the MEDHOST Customer Portal Web Site (the “Security Officer Web Site”). Please review these Additional Terms. Do not use any portion of the Security Officer Web Site if you do not agree to be bound by these Additional Terms.

By accessing any portion of the Security Officer Web Site, you are agreeing to be bound by these Additional Terms and you represent, warrant and covenant that you are currently and properly authorized to access and use the Security Officer Web Site on behalf of a current, registered customer of MEDHOST and that you will only access and use the Security Officer Web Site, or portions thereof, in accordance with such authorization, and (ii) that you will only access or attempt to access data or information for such customer and for which you have proper authorization and will only use such information or data in accordance with such authorization.

MEDHOST is not responsible for determining, granting or managing individual access rights to the MEDHOST Customer Portal Web Site or the MEDHOST software applications for a customer facility. Rather, the designated security officer of the customer is/are solely responsible for enrolling or terminating (including individuals no longer employed) an individual user’s access rights for the Web Site and/or MEDHOST software applications or portions thereof, as applicable. Certain access rights are managed by the customer security officer through the Security Officer Web Site. If you grant access rights to the Web Site, you acknowledge and agree the individual user may have access to highly confidential and sensitive information, including, without limitation, protected health and/or other similar information. Users are responsible for maintaining their passwords and other account information. See Section 5 of the MEDHOST Customer Portal Web Site Terms of Use.

MEDHOST may revise these Additional Terms. Any such revision or change will be binding and effective immediately on posting of the revised Additional Terms on our Web Site. You agree to review our Web Site, and these Additional Terms, periodically to be aware of any such revisions. If you do not agree with any revision to these Additional Terms, you should terminate your use of the Security Officer Web Site immediately.
